Medical Records

To request a report copy and/or CD of images, please bring your photo ID to our Medical Records Office Monday through Friday from 8:00am to 4:00pm. To limit your wait time, we do recommend calling ahead and requesting the report and/or CD prior to pick up. If you are picking up medical records for someone other than yourself, you must be listed on the patient's HIPAA form or provide a signed letter from the patient stating that you have their authorization to pick up on their behalf.
